To improve the quality of life in our region, by providing professional healthcare services with compassion, respect, and dignity to all.

Programs and Services

• Community Health Net, the Lake Erie region's largest Federally Qualified Health Center, provides high-quality medical, dental, vision, pharmacy, behavioral health, and other health-related services such as Health Care for the Homeless and Ryan White Part C (HIV/AIDS) programs. We also offer insurance outreach and enrollment services to all in the community. Community Health Net makes great healthcare affordable by providing access to healthcare services for all individuals, regardless of insurance status or ability to pay. In addition, we offer a Sliding Fee Discount Program based on family size, income, and nominal fees and accept all health insurers.
